2023-2024 Policies and Procedures

LOFDC - Policies & Procedures 

 

Tuition/Registration Fees:

Summer programs - Camp deposits of 50% are due upon registration and are non-refundable or transferable, with the remaining Camp balance due on the first day of camp.  A non-refundable deposit of 50% must be paid to hold dancers spot in all summer classes, with the remainder due on the first day of summer class. There are no discounts or refunds given for summer camp or dance lesson fees unless that minimum class enrollment level is not met.  

School year programs - Registration fees are $25.00 per student or $40 per family and due payable at the time of registration for our 10-month school year programs. School year tuition is due the 1st of the month. For tuition paid after the 10th of the month a $15.00 late fee will be assessed.  If tuition is still unpaid by the 20th of the month, then the late fee will increase to $20.00. Tuition not paid by the 15th of each month will result in a student being suspended from class. They may not return until the outstanding balance is paid in full.  Accounts that remain past due by the 25th of the month will result in students being permanently removed from their classes. June tuition is due paid in advance, no later than May 10th. Accounts must be paid in full by May 10th for students to perform in the June recital. All fees are per student. Tuition is payable by cash, check, card or online. Make checks payable to: Leap of Faith Dance Center (Please write student’s name in memo area of check.) Returned checks will be assessed a $25.00 fee. Tuition is calculated on a yearly fee and divided by 10 months. For this reason, tuition will ALWAYS be the same amount each month regardless of attendance.  I understand that monthly payment amounts may vary, as classes are added or dropped, or as other charges/payments are applied to my account. There are no discounts or refunds for missed classes, all sales are final for any product and/or services by LOFDC.  

 

Special Payment Arrangement Requests: 

We are UNABLE to accept any payment arrangements, for after the 15th of each month. If you need to make payments after the 15th of the month, then we ask that you pre-pay the previous month, to keep your dance account current, and your dancer eligible to attend classes. For instance, if you need to pay on the 20th of each month, you will make your September tuition payment in August. Thank you for your understanding in this matter. 

 

Costumes Fee Payments:

Ordered costumes receive no refunds. Christmas Costume Rental Fees are due by October 1, 2023, pricing is as follows: Students in one performing class $15.00. June Recital Costume fees are due by November 1, 2021. June Recital free pricing is as follows:  Lower School $80.00 per class, Combo Classes $90.00 per, Upper School & Company $90.00 per class – this fee includes recital costume, performance tights (if applicable) & minor alterations. A $15 late order fee will be applied to all costume orders that are NOT paid in full by November 1, 2023 Students joining after December 1, 2023 will be charged an extra $15.00 for shipping and handling, enrollment cut off for students participating in the June recital is January 5, 2024 and is dependent upon costume availability.  Past due accounts will not receive costumes for recital photos or the recital event until all balances are paid in full. No costumes fees are charged for summer programs.

 

Cancellations/Withdrawals:  

I understand that the above-named person(s) is enrolled in a program that requires monthly tuition, they are continuously enrolled in the program and monthly tuition charges will be incurred until I submit a “LOFDC Class Drop Request” form. This document may be obtained at the LOFDC Business Office. If I am dropping a class, IT MUST BE DONE BEFORE THE 1st OF THE MONTH. Students enrolled on the 1st of the month (or after) are required to pay the tuition for that month, in full. Upon your withdrawal, you forfeit all registration fees & tuition – there are no refunds. If withdrawing after a costume has been ordered, the costume if not picked up by parents before recital will be considered abandoned property and become property of LOFDC. In order for costumes to be released at the time of your withdrawal or for photo/recital events, your account must have a zero-balance owed. 

 

Punctuality/Attendance:

We understand that being late is sometimes unavoidable. However, any upper school student who is more than 15 minutes late for class will be asked to observe the class rather than participate. This is for safety – especially where higher level classes are concerned. A student who misses the first few exercises runs the risk of injury by not being properly “warmed-up”. **Students who are picked up more than 15 minutes after the end of their class will be assessed a $5.00 fee for every portion of 15 minutes thereafter. This charge will be directly assessed to your account. Please notify the studio business office when your student will be absent from class. 

 

Attire/Uniform:

Please refer to our Studio Dress Code for appropriate class attire. Students who do not follow dress code, will be sent home with a dress code reminder, and may be asked to sit out of class after an initial warning.

 

Substitutions/Cancellations:

LOFDC reserves the right to provide a substitute teacher if the regularly scheduled teacher is otherwise unable to teach classes. LOFDC reserves the right to cancel any class that has not met the minimum enrollment requirements. 

 

Photo Release: 

LOFDC is hereby granted permission to take photographs and or/ videos of all students to use in brochures, websites, posters, advertisements, social media platforms, and other promotional materials. 

 

Studio Communication: 

All accounts must have a current e-mail address on file by which to receive important notifications, receipts, and correspondence. 90% of our communication is via email.

 

Other:

LOFDC reserves the right to dismiss any student or family for failure to comply with studio regulations or for behavioral problems. Tuition will not be refunded. Non-students are not to be left at the studio unaccompanied by an adult. Parents must keep LOFDC informed of address, telephone, and email changes. The policies and procedures may be amended at any time without notice. Changes will be posted on the website and on the studio bulletin board. No pets are allowed in the studio.

 

Sick Policy for Dancers & Family:

If you or your dancer are experiencing any of the following symptoms, we must ask that you stay home and take a break from dance classes: runny nose • nasal congestion • sore throat • achiness • nausea • vomiting • diarrhea • fever • cough • shortness of breath or if you have reason to believe you may have been exposed to  COVID-19 for any reason. These symptoms typically appear 2 to 14 days after exposure to COVID-19. If you are experiencing any of these symptoms, self-isolate and call your Primary Care Provider (PCP) or other health care clinician for guidance.  We require that anyone entering our facility be completely symptom free from colds/illness for 48 hrs. Should a student come to dance class ill, we will contact their parent/guardian to come take them home.

 

Covid Consent Release: 

COVID-19 Leap of Faith Dance Center Screening and Consent Form
I knowingly and willingly consent to have my child participate in programs with Leap of Faith Dance Center during the global COVID-19 pandemic. I understand the COVID-19 virus has a long incubation period during which carriers of the virus may not show symptoms and still be highly contagious. It is impossible to determine who has it and who does not with the current limits in virus testing.
I confirm that my child and members of my household have not in the past 14 days had any of the following symptoms of COVID-19 listed below:

• Fever
• Shortness of breath
• Cough or any flu like symptoms including GI upset, headache, fatigue
• Runny nose
• Sore throat
• Recent loss of taste or smell

I understand that certain travel may increase risk of contracting and transmitting the COVID-19 virus. In addition, the CDC recommends social distancing of at least 6-feet for a period of 14 days to anyone who has traveled to potentially affected areas of COVID-19. Therefore, I verify that my child, nor anyone in my household, have not traveled outside the United States in the past 14-days to countries that have been affected by COVID-19.

I will hold harmless and indemnify, Leap of Faith Dance Center teachers, associates, employees, successors, assigns, legal representatives, organizers, sponsors, and supervisors, against any claims, and actions, in exchange for programs with Leap of Faith Dance Center during this Covid-19 pandemic.

Please be advised that there may be risks in being in the proximity of other people. We are taking many precautions to limit the spread of disease, yet there is still a possibility of transmission. I make this decision for my child of my own free will relying upon my knowledge and judgment of any injury they may have sustained or possible transmission of COVID-19 during participation in programs and my decision to release has not been affected by any false statements or representations pertaining to those injuries. I understand that this action is my decision. Accordingly, this agreement is not an admission of any liability regarding Leap of Faith Dance Center, teachers, associates, employees, successors, assigns, legal representatives, organizers, sponsors, and supervisors, against any claims, and actions. I have carefully read this release and understand its contents, and I am signing it of my own free act.

PLEASE do not send your child to the studio if they are sick. If your child or someone in the home has any of the symptoms above, please do NOT send your child to studio.

If you send your child to the studio, you are consenting to this form, and stating your child and any members of the home are negative for all the Covid-19 symptoms stated above. 

 

Waiver and Release & Financial Agreement: 

 

I recognize the risks of illness and injury inherent in any exercise/dance program and am participating in the studio’s program upon the express agreement and understanding that I am herby waiving and releasing the studio and its agents, representatives, instructors, and anyone else acting in furtherance of its programs, from all liability for any injury or condition resulting in his/her participation in the school’s programs. I authorize Leap of Faith Dance Center to make emergency medical decisions if I am not able to be reached in a timely manner.  I also understand that I (the person signing this contract) am responsible for paying tuition on the first of the month and understand that I will not receive monthly statements. I understand that school year Tuition not paid by the 15th of each month will result in a student being suspended from class.  Accounts that are past due by the 25th of the month will result in students being permanently removed from classes. Students may not return until the outstanding balance is paid in full.  I understand that I am responsible for paying Camp deposits of 50% of the Camp balance that is due upon registration and is non-refundable or transferable, with the remaining Camp balance due on the first day of camp.  And that a non-refundable deposit of 50% must be paid to hold dancers spot in summer class with the remainder due on the first day of summer class.  I understand that there are no discounts or refunds for missed classes, all sales are final for any product and/or services by LOFDC.

 

I have read this complete brochure and understand its contents and agree to participation as outlined.
